Position Summary:

The Peer Health Coach provides wellness training and support to families in the Primary Care Behavioral Health Integration (PCBHI) Program and facilitates skill-teaching groups. This position also provides information on wellness and will promote opportunities to enhance the ability of families served to improve their health and wellness .

• Schedule: Monday - Friday, 8:00 am - 4:00 pm Location: Fairfax County (Reston, VA).
• Hybrid: 3 days in office, 2 days Work from Home.

In this full-time Peer Health Coach role, you will:

• Coordinate and complete health assessments within tight deadlines;
• Complete client notes/records as required;
• Conduct support groups focused on health maintenance and prevention on medical disorders;
• Provide education on the recovery process and promote opportunities to enhance the ability of consumers served to move into recovery; and
• Arrange crisis intervention as needed.

You are a great fit for the Peer Health Coach role if you have:

• Must be a Peer which is a person who has personal lived experience and has used mental health services (past or present)
• High school diploma or GED
• Certified Recovery Peer Specialist (Preferred)
• Ability to teach disease management and health maintenance curriculum
• Comfortable working with both adults and children
• Ability to utilize supervision to clarify clinical concerns
• Knowledge and skill in computer procedures relevant to this position.
• A valid driving license with a clean driving record is required
• A high degree of empathy toward individuals with behavioral health disorders and the ability to connect with individuals of diverse ethnic and cultural backgrounds
